Requirements or Credentials


  1. 1. RN & Driver's license, SSC (front and back copy)
  2. 2. BLS, ACLS, PALS, NRP, MAB (front and back copy)
  3. 3. AWHONN Advance Fetal Monitoring
  4. 4. Physical Exam (valid within a year)
  5. 5. Chest X-ray (valid within 4 years) or PPD test (valid within 1 year)
  6. 6. 10 Panel drug test (valid within a year)
  1. 7. Mask fit (3M 1860)
  2. 8. Fire card for Los Angeles County hospitals
  3. 9. Flu vaccination or declination
  4. 10. Hep B Positive Titer or declination
  5. 11. MMR & Varicella Titer or Immunization

PROCESS
